Grade 2 Gynecomastia Case Study: Achieving a Masculine Chest Contour

In this recent case, my patient—a highly athletic individual—struggled with chest fullness that persisted despite his intense workouts. He developed gynecomastia about five or six years ago, leading to a buildup of breast tissue that created a noticeable heaviness in the lower chest. This case required a specialized approach to create a natural, more masculine contour, and I’ll walk you through the key steps of the process at my surgical facility in Beverly Hills.

What Is Grade 2 Gynecomastia?

For those who are not familiar, grade 2 gynecomastia is characterized by breast tissue that extends beyond the areola border. But, doesn’t significantly impact skin elasticity, allowing for a good chance of skin retraction after surgery. In this patient’s case, the skin condition was ideal, which allowed us to proceed without any skin removal. I performed a pinch test to assess the density and extent of the breast tissue. His chest had a much thicker layer in the lower portion, confirming the presence of dense breast tissue that could be addressed surgically.

The Surgical Plan: VASER Liposuction and Breast Tissue Excision

The approach for this type of gynecomastia correction combines VASER liposuction and direct excision of the glandular tissue. VASER liposuction was the first step, helping me target and remove deep fat through small, discreet incisions. Using this method allowed me to create a more even chest contour by breaking down fat more precisely, which is especially important for this procedure. Once the fat was removed, I accessed the breast tissue through a small incision, carefully excising the gland to achieve a flatter chest contour.

Ensuring Balanced and Natural Chest Contours

One of the most crucial steps in this grade 2 gynecomastia surgery was maintaining natural chest contours. To do this, I marked specific transition zones where the upper abdomen meets the chest. Creating these seamless transitions prevents any unintended “bulging” effect between areas with and without glandular tissue. By carefully planning and marking these regions, I was able to achieve a balanced look, ensuring that the chest would match the rest of his athletic build.

Post-Surgery Transformation

Once the procedure was complete, we had removed approximately 350cc of fat. However, the main component contributing to his gynecomastia was the dense glandular tissue, which we successfully removed as well. After sitting him up to assess the results, we could see that his chest already displayed a much-improved contour, with significantly reduced projection and a smoother transition from his chest to the upper abdomen.

The result for this patient in Beverly Hills was a leaner, more defined chest that now matches his fit frame. Gynecomastia surgery is a transformative procedure that can make a big difference for patients, especially those who’ve worked hard to maintain a healthy lifestyle but struggle with stubborn breast tissue.
