There is a moment in nearly every consultation when a patient says the same thing: “I’m lean everywhere else… so why does my chest still stick out?”

This was exactly the case for a young man who recently came to see me. He had developed gynecomastia during puberty, the most common time for it to appear, and even years later, despite staying lean, his chest never flattened. From the front, it looked like muscle. From the side, it told a different story.

This is the part of gynecomastia that exercise cannot touch.

The Difference Between Fat and True Gynecomastia

When most men look at their chest, they assume the fullness is fat. But true gynecomastia is caused by glandular breast tissue, not body fat, and it behaves very differently.

During this patient’s video consultation, I could already tell he had grade two gynecomastia. That means the gland extends beyond the borders of the areola, but the skin and nipple position are still normal. It creates projection, not sagging.

But video can only tell you so much.

When the patient came into the office for his in-person exam, the real story became clear. The pinch test showed thick tissue right under the nipple, even though the surrounding chest was lean. The gland did not feel like a single hard lump. Instead, it blended into the fat around it, which meant a simple cut-and-remove approach would not be enough.

This is where experience changes the outcome.

Why Liposuction Matters in Gland-Dominant Gynecomastia

Many people think gynecomastia surgery is just about gyno gland removal – cutting out a gland. But when the border between fat and gland is blurred, liposuction becomes an essential tool.

In this case, I used liposuction first, not to remove large amounts of fat, but to sculpt the chest and separate the gland from the surrounding tissue. This makes the gland easier to identify and safer to remove cleanly.

After liposuction, something important happens: The outer chest starts to thin, but the center remains thick. That tells me exactly where the gynecomastia lives.

No amount of liposuction can make that central thickness go away because it is not fat. It is breast tissue.

What Grade Two Gynecomastia Really Looks Like

Grade two gynecomastia is not just about size. It is about architecture.

Before surgery, this patient’s chest created a shallow angle where the chest met the abdomen. Instead of sloping upward into the pectorals, it pushed outward at the areola. Even lying flat on the operating table, his chest still projected.

That is one of the most reliable signs of true gynecomastia.

When I removed the gland, the transformation was immediate. The outer border of the pectoral muscle appeared. The chest flattened naturally. The angle between chest and abdomen widened, giving him a more athletic, masculine silhouette.

This is what surgery does that no workout ever can.

Why Lean Men Still Get Gynecomastia

One of the most frustrating parts of gynecomastia is that it does not care how fit you are.

This patient was lean. Yet when the gland was removed, it covered nearly the size of a hand. That is why dieting, cutting, or bulking never fixed the problem. The tissue was not supposed to be there in the first place.

Once the gland is removed, the chest finally behaves like a male chest should.

The swelling from liposuction fades. The skin contracts. The compression garment helps everything settle into place. And over the following weeks, the chest becomes smooth, flat, and proportional to the rest of the body.
