Grade 2 Asymmetric Gynecomastia: Lipo and Excision of Glandular Tissue

As a plastic surgeon, I’ve encountered the complexities of gynecomastia in various forms. One intriguing case that comes to mind is a grade 2 gynecomastia, where the condition manifested unevenly, presenting unique challenges. In my latest youtube video, I walk you through how a blend of gynecomastia liposuction (chest fat removal) and glandular tissue excision resolved asymmetry and resulted in well-defined chest contours, offering my patient a renewed sense of confidence and pride.
Understanding the Challenge
This patient’s journey began during his teenage years when he developed gynecomastia. A closer examination revealed that he presented with an asymmetric case, with the right side being larger and positioned lower. This condition, technically classified as grade 2 with elements of grade 3 due to skin overhang, posed both aesthetic and functional concerns for him.
The Surgical Approach
The surgical strategy was a two-fold process. Firstly, I marked the target areas for liposuction. The objective was to sculpt the chest, not only addressing the fatty tissue but also focusing on the lateral chest region to achieve a more defined pec contour. This approach aimed to reduce the bottom heaviness of the chest.
However, it’s important to note that gynecomastia is not solely a matter of excess fatty tissue. After the gynecomastia liposuction, I proceeded with a small incision at the base of the areola to remove the surplus glandular tissue responsible for the condition. This dual approach ensured a comprehensive and balanced outcome.
In the Operating Room
The patient’s asymmetry was evident even as he lay on the operating table. The conical protrusion around the peak of the nipple was a clear indicator of the challenge at hand. To initiate the corrective process, I began with Vaser liposuction for chest fat removal. Subsequently, I addressed the glandular tissue to restore a more natural chest contour.
Restoring Symmetry and Balance
The transformation achieved through the procedure was striking. The asymmetry that had plagued the patient was corrected. The Vaser liposuction and precise glandular tissue removal produced a chest that closely matched the patient’s desired contour.
Notably, the incisions made were discreet, ensuring minimal scarring. The reduction in glandular tissue also led to a proportional reduction in the size of the areola, contributing to a more harmonious chest appearance.
A Positive Outcome
The surgery not only addressed the physical concerns but also had the potential to restore his self-confidence and quality of life.
