Gynecomastia in Bodybuilders
Written by Dr. Babak Dadvand
As a plastic surgeon specializing in gynecomastia surgery, Dr. Dadvand sees a variety of causes of increased breast tissue in men. One of the more direct and easily identified causes of gynecomastia is exogenous steroid use.
Grouped broadly, there is a large variety of steroid medications available and routes of use, from gels to injectables. Although not exclusive to this patient population, breast enlargement due to steroid use is the number one reason why bodybuilders come to Dr. Dadvand for gynecomastia surgery in LA.
What Causes Gynecomastia in Bodybuilders?
The human body produces hormones, which are basically messengers that are sent from one part of the body to another and direct that body part to perform a specific function. Hormones work in a delicate balance, therefore, having too little or too much of a hormone can cause specific symptoms.
In the case of testosterone, the body maintains a certain level of free testosterone. Too little testosterone can make men feel tired and result in decreased libido. However, when men take exogenous testosterone (and their derivatives), it can lead to excess testosterone that then gets converted into estrogen. This leads to excess estrogen levels, which can stimulate the breast tissue to grow, leading to signs of gynecomastia.
Overview of Gynecomastia Surgery for Bodybuilders in Los Angeles
There are medications, commonly referred to as estrogen blockers, which may counteract any effects of excess estrogen in the body. However, even on an estrogen blocker, men can still develop gynecomastia.
Gynecomastia surgery for bodybuilders involves removing the tissue that causes the chest to look puffy or enlarged. In most cases, this involves removing 100% of the breast tissue. However, sometimes a very small amount of tissue is left behind to avoid any contour abnormalities. For this reason, it is important to discuss with patients that if they continue steroid use after surgery there is a chance that gynecomastia may return, albeit to a lesser degree.

What is the surgery for gynecomastia?
Gynecomastia in men can be addressed through male breast reduction. Also known as gynecomastia surgery, this procedure aims to remove all or most of the glandular tissue that is causing the chest area to look bigger. It is a cosmetic procedure and it results in a flatter, firmer chest.*
Can you get gynecomastia again after surgery?
All patients should understand that there is always a slight chance of developing gynecomastia again after surgery. This happens rarely and Dr. Dadvand has only observed this phenomenon in patients that experience excessive weight gain or take certain types of steroids and medications that produce breast enlargement.
Is gynecomastia surgery safe?
Gynecomastia surgery is a relatively safe procedure. However, like other cosmetic surgeries, there are some risks and potential side-effects you should keep in mind. These include complications with anesthesia, a minimal chance of infection, and discomfort during the recovery.
Does insurance cover gynecomastia surgery?
It is important to note that gynecomastia surgery is considered a cosmetic procedure. As such, most insurance providers will not cover the procedure.
How many days rest do I need after gynecomastia surgery?
Gynecomastia surgery requires an incision and the removal of glandular tissue, so you will need to take some time off work and school for the procedure. You should be able to return to regular activities within a few days, but you will need to wait at least two to three weeks to start heavy physical activity.
